In the Academic Year 2003-04, the College of Arts and Sciences established the Jimmy Smith Outstanding Service Award in recognition of the numerous contributions of Jimmy Smith. Full-time Arts and Sciences faculty (including emeriti) with at least five full years' employment at ASU are eligible for nomination. Falculty self-nomination is permissible.

All Arts & Sciences faculty (including part-time and emeriti), staff, and alumni are eligible to submit written nominations. Please complete the attached nomination form, addressing the following:

• Meaningful benefit and significant effect of the nominee's service to the college, the university, and/or their profession.
• Consistent outstanding service contributed over multiple years.
• Leadership and initiative demonstrated by the nominee in their service.

Nominees will be notified in writing and asked to submit supporting materials. These materials will not be returned unless requested by the nominee. The selection committee, composed of the most recent recipient, three Arts & Sciences faculty members, and the Associate Dean will evaluate each completed nomination package according to the criteria above.
